How do I change my address?
Please let us know as soon as possible if you change your address or any other details, e.g. name, telephone number. It is essential that your Doctor has correct, up to date information in the event that we need to contact you.
There are several ways that you can inform the surgery that you have changed your address. The first way is that you can write in to the surgery giving your name, new address and telling us as to whether there are any other members of your household (giving their full names) so that we can change their address too.
Another way is that you can come into the surgery where one of the receptionists will take your details.

If you have recently changed your address and are currently being followed up by a hospital, please remember to let the hospital have your new address so that appointments for you are not sent to the wrong address.
