Medical Certificates
There is no need for you to consult your GP to obtain a self
certification of illness (SC1) form for your first five working days of
sickness.
If you require a sick note within the self certification time period for
insurance purposes or your employer then a private note will be issued
and this will incur a charge of £10.00
The New Fit Note replaces the old Med 3 / Med 5 certificates.
